Overview

Somatus is a value-based care company that manages kidney and, more recently, heart disease for health plans, providers, and at-risk medical groups. Founded in 2016 and headquartered in McLean, Virginia, the company built a model that pairs local, in-person care teams with data and technology to identify at-risk patients earlier, slow disease progression, and shift dialysis starts toward the home rather than in-center clinics.

The company contracts with health plans and provider groups on a risk-sharing basis, taking on accountability for the total cost and quality of care for members with chronic kidney disease and end-stage renal disease, and has extended the same model to congestive heart failure. Somatus says its programs operate across all 50 U.S. states and Washington, D.C., working with health plan partners, self-funded employers, and thousands of nephrology and other specialist providers.

Somatus has raised financing from a mix of healthcare strategics and institutional investors, including a 2022 round that press coverage pegged at a roughly $2.5 billion valuation, and has been recognized by NCQA for case management, population health, and health equity programs.
